Long-term care options counseling is consultation and advice about the options available to meet an individual’s long-term care needs, including enrollment into long term care options.
Wisconsin’s long term care programs, Family Care and IRIS, provide services and supports to eligible adults to meet their long term care needs, allowing them to maintain independence and choice in their lives.
Current long term care programs are designed to promote:
• Access – creating a setting where people are able to receive the services they need when they need them.
• Choice – allowing individuals to choose where and how they receive their care.
• Quality – ensuring that services meet an individual’s needs and provide for a good quality of life.
• Cost-effectiveness – assure a sustainable future.
Am I eligible?
• You must be a resident of the county in which you are applying.
• You must be elderly or have a physical or developmental disability.
• You must be found functionally eligible through a long term care functional screen.
• You must be financially eligible
How to apply or receive additional information
Contact the local office of the Aging & Disability Resource Center (ADRC) of Barron, Rusk & Washburn Counties and ask to speak with an Information & Assistance Specialist. ADRC’s are the single point of entry for providing information and options about long term care programs as well as determining eligibility. An Information & Assistance Specialist will meet with you to determine your eligibility for long term care programs as well as assistance with the enrollment process.
